Program Description: American Cancer Society (ACS) is the largest voluntary health organization dedicated to eliminating cancer as a major public health problem by preventing cancer, saving lives from cancer, and reducing suffering from cancer through research education, advocacy and service.  The following are ACS projects that have Nutrition and Physical Activity as their primary focus are: April Nutrition & Exercise Campaign, Community outreach to Special Populations (African American and Latino), Body and Soul, for corporations, companies and organizations; Meeting Well, & Active For Life, and for youth; Generation Fit, and Spring Into Health Day.  The theme of these programs is to promote to adults and youth that by exercising for 30 - 60 minutes a day, 5 days a week and eating the right foods may lower the risk of certain cancers. We also are strong advocates for Comprehensive School Health programs, and work on many levels to promote school health policies, including our newest program, the Healthy Kids Network.  This program communicates to parents how to keep their kids healthy and improve their school health education polices.

Program Description: Baja Health Clinic is a privately owned company founded 15 years ago.  Its owner and manager since its conception, has been Dr. Alma Pedrote.  The company markets weight reduction and exercise plans, and gerontology services to the community.  Recently, the company has started providing health care seminars, that are sold to Mexican corporations, especially to those in the manufacturing sector.  One of the goals of such seminars, is to educate the workers about the benefits of adopting quality nutrition habits.
Baja Health Clinic has two main projects for the year 2001:
1 - The first is to provide US corporations, specifically those within the health care and biotechnology sector of San Diego, the facility to conduct research and experimentation studies in Mexico, for the purpose of testing new medications, new foods or new exercise equipment, before market.
2 - The second is to create the Clinical Institution for Diabetic Men.  This institution will be in part financed by fees paid by the patients that subscribe to the services of the clinic.

Program Description: Our mission is to reduce the burden of diabetes in California's diverse communities.  To pave the path for improved outcomes for people with and at risk for diabetes, we take a multi-pronged approach.  Our work is guided by national initiatives, statewide goals, as well as, community level input.  It ranges from supporting system-wide improvements in health care delivery, to developing peer-to-peer education programs led by community volunteers.  We promote proven methodologies including the Chronic Care Model and the team approach to care.
The Diabetes Program supports primary prevention efforts that engage industry, building and community planning, schools, and workplaces to improve access to healthy foods and daily physical activity.  In particular, the CDP works to increase primary prevention efforts in health care delivery systems such as medical groups and health plans.

Program Description:
CRC is a social service agency serving the north coastal communities of San Diego County.  CRC provides food security for low income and homeless families.  Bread and other foodstuffs are provided to the community six days a week through the bread room.  CRC maintains a government funded Commodities Program, which are distributed every third Tuesday of each month at three CRC operated locations.  Each holiday season CRC hosts the Holiday Basket Program in which food, clothing and gifts are provided to seniors, homeless, unemployed, working poor and victims of domestic violence.  CRC collaborates with the WIC program and with the Encinitas and Carlsbad Farmer’s Market to provide free food to program clients.  In addition, Libre, CRC’s emergency domestic violence shelter, participates in Nutrition Now, a specialized gardening and nutrition education program.  Nutrition Now also provides community education to all CRC clients on “5 a Day” food choices, the food guide pyramid and the importance of preparing healthy meals.

Program Description:
The Division of Community Pediatrics at the University of California, San Diego is the lead agency for the 5 a Day Campaign in the San Diego and Imperial County Region.  Our mission is to conduct a regional public health campaign that increases the consumption of fruits and vegetables to a minimum of 5 servings a day, as part of a healthy lifestyle, in an effort to reduce the risk of chronic disease.  A secondary aim is to increase physical activity to 30 and 60 minutes a day for adults and children, respectively. Two campaigns comprise the 5 a Day campaign, including: 1) "The Latino 5 a Day Campaign" which targets California's limited income Latino adults through mass media, retail, and community channels, and 2) "The California Children's 5 a Day-Power Play! Campaign" which targets 9 to 11 year old children and their parents through mass media, schools, supermarkets, farmer's markets, youth groups, food service/restaurant, and other community and health organizations.  P
ower Play! is a free bilingual nutrition education program aligned with state standards.

Kalusugan is a non-profit, multi-service organization that has been providing high-quality, professional services that strengthen Filipino families and their communities.
Currently we are working on the Healthy Eating Campaign that targets the National City area to improving access to healthy choices in restaurants and grocery stores.  The project is funded for two years.
The mission of KCS is to improve the health and well-being of Filipino Americans and other underserved groups through community outreach, research, training and advocacy.  It was founded in 1992 as Project Kalusugan and was incorporated as a 501 (c) 3 organization in 1995 under its present name.  In 2002, it established a Filipino American Wellness Center (FWC) in National City.  The aim of the center is to integrate community services and design a comprehensive health promotion program for FilAms.  The community outreach component includes the following activities: Screening and Referral Services, Nutrition and Physical Activities, Community Health Promotion Seminars, and Parent/Youth Workshops.  We also have socials such as karaoke nights and parties to celebrate special occasions.  All of our programs are intergenerational and involve collaboration with many agencies.  We have a monthly oversight committee meeting and quarterly board meeting.  Visit our Website to see our schedule of activities.

Community Development:
Los Niños defines Community Development as a participatory process where community members identify community needs and organize themselves to take the actions necessary to improve their quality of life.  To achieve the mission, Los Niños is organized through four strategic programs: Family Health and Food Security, Family Financial Security through Micro-loans, Community Development Education and Service and Education-based Travel.  Our Family Health and Food Security program:  Community food security is defined as "all people in a community obtaining a culturally acceptable, nutritionally adequate diet through non-emergency food sources at all times".  Los Niños integrates nutrition, health and organic gardening into an approach that offers community members self-reliant methods to address their food needs.  Community outreach workers are trained to teach in their own neighborhoods disseminating information about nutrition, health and organic gardening.  The Los Niños outreach workers ("promotoras") are community members, generally women, who are residents in the low-income teacher in their communities.  For this work they receive a stipend, transportation funds and teaching materials.  This strategy has proven to be effective, reaching many families in this complex border region.  The needs are many: access to water, electricity, adequate housing and, most basic of all, sufficient nutrition for the children and their families.  As these community classes continue, in the process the groups get organized and seek to address other issues in the community.  There are now 60 trainers providing education reaching 18,000+ community members annually, either directly or through their families.

Program Description: The San Diego Diabetes Coalition is a fairly new collaborative with more than 80 members representing at least 25 agencies in the San Diego county area.  The mission of the Coalition is to improve the health and well-being of San Diego county residents at-risk or living with diabetes by facilitating high quality diabetes care that includes education and prevention.  Currently, the Coalition is completing its first strategic planning process.  So far, four action committees have been identified and are in line with the State of California's Diabetes Programs' goals: Access to Care, Policy, Quality of Care and Prevention.  In 2006, the Coalition plans to continue fine tune its organizational development and create opportunities for their member agencies to enhance their services and collaborative efforts with the outcomes of increasing better self-management of diabetes and subsequently, decreasing the incidences and costs of high-end medical complications.

Program Description: We conduct health promotion research projects through San Diego State University.  My current project is a school and community-based physical activity research project aimed at preventing the decline in physical activity levels of adolescent girls (middle school aged).  One proposed research project would track these middle school girls into high school to better understand the pattern of physical activity and nutrition behaviors.  Another proposed project would promote healthy shopping among Latino customers in grocery stores.

Program Description: Southern Indian Health Council, Inc. (SIHC), a nonprofit corporation, was founded in 1982 by a seven tribe consortium.  The goal of the Nutrition Services Department of SIHC, in partnership with the Nutrition Network, is to provide nutrition information on the importance of eating healthy with an emphasis on the 5-A-Day principles; living a healthy lifestyle; cooking and shopping classes; as well as, encouraging physical activities for our target population.
Since many of the residents of our target population are located in extremely rural areas, the Nutrition Services staff participate in a wide variety of community based events such as, Health Fairs, the Commodity Foods Program, After-School Programs, Welfare to Work Program, Public School curriculums and Pow Wows.  We also publish monthly newsletters, one for adults and one “Just For Kids.”
Interactive classes designed for all ages, help participants to learn how to prepare healthy snacks/ meals, individual modules include: for those on the go, cooking within a budget, foods for fuel and cut the fat, to name a few.  Hands-on cooking classes and food tastings are provided to various age groups and encouragement of physical activity is always included in our programs.

Program Description:
The Sweet Heart Project was developed by the California Black Health Network in response to the CDC/REACH 2010 initiative “Eliminating Health Disparities Through Partnership”.  The name “Sweet Heart” refers to the focus on diabetes, which many African Americans refer to as “sugar”, and on diseases that affect the heart.  The San Diego Black Health Associates “Sweet Heart Project” was funded in two parts: Phase I developed a community action plan which focused on developing partnerships with community based agencies.  Phase II calls for the implementation of the community specific action plan that was developed by the Sweet Heart collaborating partners in the Phase I planning year.

Program Description: The Adult Family Nutrition Program consists of EFNEP and FSNEP, free programs providing workshops designed to assist limited income families with young children and food stamp recipients to improve their diets through nutrition education.  Nutrition Education Assistants trained by the University of California teach groups of families about nutrition, meal planning, food buying, foods safety and storage, and how to stretch their food dollars.  Our Home Study Family Nutrition Program is a learn-by-mail program designed to assist families that cannot attend the workshops.
     The Youth EFNEP Program trains adult volunteers, parents, teacher and community agencies' staff, in groups of 1 to 30 to teach nutrition classes to youth.  The youth program provides free training and curriculum filled with fun activities for youth between the ages 6-18, available in English and Spanish.  For information or to schedule a training class, please call Stella Taylor at 858.694.2122.

Program Description: The Healthy Eating Program (HEP) is a part of an ACS funded grant, in partnership with the UCSD Cancer Center.  The goal of HEP is to establish healthy eating habits within individuals, and specifically to increase fruit, vegetable, and whole grain consumption.
